This directory provides a list of Signature Advantage Plan’s and Signature Advantage Community’s current network providers for Signature Advantage Plan in Kentucky and Signature Advantage Plan in Tennessee. To request a hard copy of Signature Advantage’s provider directory, please call our Customer Service Department at 1 (844) 214-8633; TTY 711, April 1st – September 30th: Monday – Friday- 8:00 AM to 8:00 PM | October 1st – March 31st: 7-Days-a-Week- 8:00 AM to 8:00 PM. Signature Advantage will mail a hard copy of the provider directory to you within three (3) business days of your request. Signature Advantage may ask whether your request for a hard copy is a one-time request or if you are requesting to receive the provider directory in hard copy permanently. If you request it, your request for hard copies of the provider directory remains until you leave Signature Advantage or request that hard copies be discontinued.
