Signature Advantage Plan (HMO ISNP) and Signature Advantage Community (HMO ISNP) are an HMO plan with a Medicare contract. Enrollment in Signature Advantage (HMO ISNP) and Signature Advantage Community (HMO ISNP) depends on contract renewal. This plan is available to anyone with Medicare who, for 90 days or longer, has been a resident or expects to be a resident of a long-term care (LTC) skilled nursing facility (SNF), LTC nursing facility (NF) or (ALF) assisted living facility in the Signature Advantage network.
Signature Advantage (HMO ISNP) has been approved by the National Committee for Quality Assurance (NCQA), a non-profit organization dedicated to improving health care quality, through December 31, 2021. Signature Advantage Community (HMO ISNP) has been approved by the National Commitee for Quality Assurance (NCQA), a non-profit organization dedicated to improving health care quality through December 31, 2022.
