Your Edge to Better Healthcare

Signature Advantage plan members get more than what is covered by Original Medicare, with a dedicated nurse practitioner leading a personalized care plan. Signature Advantage is a Special Needs plan that covers all the benefits of Original Medicare (Parts A and B) with prescription drug coverage (Part D). If you or your loved one is eligible, Signature Advantage may be the extra benefit you need to maintain and enhance your health and well-being. Some of the benefits include specialized chronic disease care coordination, preventative screenings, and routine vision and dental coverage.

Service Area

Signature Advantage Plan is a unique plan that can enroll Members year-round. The plan focuses on Members who meet residential requirements in the Signature Advantage participating nursing facilities located in the following Kentucky and Tennessee counties:

Within these counties, you must live in one of our long-term care nursing centers to join this plan.  Please call us at (844) 214-8633 or (TTY 711) to locate one of our facilities.

Signature Advantage covers all medically-necessary and preventive services covered under Medicare Part A and Part B, and prescription drug coverage under Part D.

Signature Advantage was created in 2016 to deliver compassionate care, specially tailored for those Members residing in a nursing facility.

Our goal was to create a Medicare Advantage Plan that meets each individuals’ clinical needs and provides preventative, coordinated quality healthcare.

Our interdisciplinary team of clinicians and innovative services provide our members with an Edge to Better Healthcare.
